Coordinated Care for Children with Medical Complexity (CCCMC) Program
Advocate Children’s Hospital is dedicated to caring for all children and recognizes that children with multiple medical problems need a special kind of care.
The Coordinated Care for Children with Medical Complexity (CCCMC) program is a new model of care designed to treat children with complex medical conditions by helping them navigate the complicated healthcare landscape. In the Advocate Children's Hospital CCCMC program, we assist families by arranging care between hospitals, primary care doctors, pharmacies, therapists and other health care providers.
The CCCMC program uses a team-based model across specialties, improving the level of care to help children with chronic illnesses have an improved quality of life by guiding families through difficult healthcare experiences. All members of the broad inter-professional care team work together with community resources (such as transportation, therapy and pharmacies) to enhance health outcomes for children through coordinated care and increased accessibility.

We offer primary care as well as inpatient and outpatient consultation depending on the needs of your child and your family, as well as the recommendations of the referring provider(s).
