Preparing for your child's surgery

Find a pediatric surgeon

At your first appointment, you’ll learn how our family-centered approach to pediatric surgery may help your child. Remember to bring a health insurance card for your child and photo identification of the parent or legal guardian who will accompany them.

You’ll also need to bring following information about your child:

  • Referring physician information, including the name, address and phone number of their primary care provider
  • Imaging records (reports and films for all imaging studies performed outside of Advocate Health Care)
  • Past medical history, including any prior medical conditions and the details of any previous surgeries and/or hospitalizations
  • List of medications (including herbal and over-the-counter)
  • List of allergies

Having surgery can be tough for anyone, especially our littlest patients. That’s why we’re here to help prepare you and your child for what to expect throughout the surgery process.

Our experienced team of nurses and nurse practitioners provides compassionate care so you can feel comfortable asking any questions about your child’s care and treatment. Please call us if you have additional questions.

  • 24/7 accessOur team is available by phone from 7:30 a.m. to 4 p.m. Monday through Friday. If you call during off hours and have an immediate concern, our 24-hour answering service can connect you with our on-call surgeon.
