Using your vaccination schedule and record
Your child's vaccination record and schedule will help you keep track of their vaccines. Here are some tips for keeping track of your child’s vaccines:
- Review the schedule: Save or print a copy of the vaccination schedule [PDF]. The yellow boxes tell you when your baby needs their shots.
- Save or print your child’s record: Keep a copy of the immunization and developmental milestones record [PDF]
- Review with your doctor: Take these records with you to all doctor visits. At each visit, review the record with your doctor. When your baby receives a vaccination, ask your doctor to date and sign the record.
- Store records or sign up for digital access: With proxy access to your child’s account, you can have 24/7 digital access to their medical records in LiveWell.
