The best way to make an appointment is to call the Advocate Contact Center 24 hours a day, 7 days a week. Our center understands the time-sensitive nature of many diagnoses and conditions and can help secure an appointment for you as quickly as possible.

What to bring to your child’s appointment

• Referring physician information, including the name, address, and phone number of your pediatrician
• Imaging records (e.g., recent CT scans, MRI scans on CD as well as reports)
• Immunization records
• Past medical history, including any prior medical conditions and the details of any previous surgeries and/or hospitalizations
• List of medications (or medication bottles)
• List of allergies, including previous reactions
• Insurance and identification information (including Social Security Number)
• Items to keep your child occupied while waiting (e.g., books or small games)
